Referrals
Self referrals are accepted.
Referrals from health professionals, including medical specialists:
Referrals are also accepted from health professionals and medical specialists. Supporting documentation is preferrable.
General Practitioner referrals:
Standard referrals from GPs are accepted as above.
For referrals that attract Medicare rebate (chronic conditions and/or Type 2 Diabetes into group program) please provide all documentation as required by Medicare. See 'rebates' for more details or look up a step-by-step guide on how to make a recognised referral. Under the EPC plan, GPs can use the Allied Health Referral form (can be downloaded from www.health.gov.au/strengtheningmedicare).
Referral information can be faxed or mailed to Mari prior to client appointment, or may be given in person by the client on the day of consultation.
